Can family be involved in care?
Yes. Family members and caregivers are encouraged to be involved in patient’s care as much as possible, based on patient’s needs, preferences, and consent. Home health care often includes teaching and guidance for family members or caregivers so they can better understand patient’s condition, safety needs, medications, diet recommendations, warning signs to report, and when to seek emergency help.

Can I discontinue home health care services?
Yes. Patient has the right to discontinue home health care services at any time.


What if home health care is appropriate for me and I would like care from your agency, but my doctor will not send a referral to your agency?
If you think home health care services may be needed, please call us. Our team can review your needs, explain the referral process, and help guide you on possible referral options when appropriate.

Keep Important Health Information Organized
Keep important records in one place so they are easy to find when needed. This may include diagnoses, medication lists, allergies, physician instructions, care plans, discharge papers, insurance information, emergency contacts, and advance care documents.
Track Medications and Health Changes
Use a notebook, binder, calendar, or phone reminders to track medication schedules, refill dates, symptoms, side effects, blood sugar or blood pressure readings if ordered, and any changes in condition that should be reported to the physician or care team.
Communicate Clearly With Providers
Write down questions before appointments, take notes during calls or visits, ask providers to explain anything that is unclear, and keep copies of instructions or updated care plans.
Make Home Safer
Reduce fall and injury risks by removing loose rugs, keeping walkways clear, improving lighting, using non-slip mats, securing or relocating cords away from walking areas, and placing frequently used items within easy reach. Keep exits unobstructed and make sure smoke alarms are working properly.
Create a Care Routine
A daily routine can make care easier to manage. Plan times for medications, meals, hygiene, rest, therapy exercises if ordered, appointments, and other care needs.
